Intake forms replace the clipboard: new-client consultations, color consent, extension agreements, patch-test waivers. Clients fill them out on their own phone, and the answers land on their profile instead of in a filing cabinet.

Build a form

Go to ClientsForms and create a new form (or CreateNew Form from anywhere). Give it a name, write a short welcome description for the first screen, then add your fields. The field types cover the whole clipboard:
  • Short Text and Long Text for open questions
  • Dropdown and Multi-Select for pick-lists
  • Checkbox, Date, Phone, and Email
  • Photo Upload, for inspiration pictures or current-condition shots
  • Signature, drawn with a finger
  • Consent Agreement, which pairs your exact consent language with an explicit agreement
Mark the fields that can’t be skipped as required. When a client finishes, they see a simple thank-you screen, and you see the response.

Publish and share

A form starts Inactive so you can build in peace. When it’s ready, Activate form, then Copy public link. That link is the whole distribution strategy:
  • Text it to new clients before their first visit
  • Put the consultation form behind your Instagram link
  • Keep a tablet at the front desk open to the waiver
Need to pull a form from circulation? Deactivate form kills the link without deleting the form or its responses.

Where responses go

Every submission lands on the client’s profile under the Forms tab, next to their appointments and history. Open a response to read every answer, exactly as submitted. Signed consent stays attached to the person who signed it, which is precisely where your future self wants it.

Forms that earn their keep

  • New client consultation: hair history, goals, how they found you.
  • Chemical service consent: your language, their signature, every time.
  • Patch test acknowledgment: short, specific, dated.
